Local Office Closing July 21st, 2017

Dear Valued Patients,

 

Horizon Natural Medicine will be closing on July 21st, 2017. On this date, I am moving my practice out of the Phoenix area.

 

While the physical location of my practice is changing, I will still be available to all patients through telemedicine visits should you desire to continue your medical care with my office. After July 21st, you can simply call or text the office to request a phone or video chat (Skype) appointment where we can follow up in a similar fashion as we would if you were visiting my office in person. Charges for telemedicine follow ups will be as follows: $65.00 for visits less than 30 minutes, $75.00 for 30-45 minutes, and $85.00 for 45 minute-1 hour visits. I also plan on returning to the Phoenix area every 6-12 months to follow up with patients who would like to continue under my care, but unfortunately, I cannot guarantee this at the current time. Please make sure that you subscribe to my email list so that you can be informed of upcoming dates and times of in-person appointments.

 

For patients who would prefer to continue their medical care with a local naturopathic doctor, I strongly recommend Dr. Shawna Eischens and Dr. Thomas Kruzel at Rockwood Natural Medicine in Scottsdale, AZ. Their office number is 480-767-7119. While I realize that this office will be a longer drive for some patients, these two are highly competent and skilled physicians who practice in a very similar fashion to myself. As all my patients know, homeopathy is a very large part of my practice and it is something that not many naturopathic physicians in the Phoenix area specialize in. Both Dr. Kruzel and Dr. Eischens are skilled homeopaths who also have extensive experience in treating a large variety of both acute and chronic medical conditions through natural modalities. To obtain copies of your medical records, you may call the office at 602-359-2893 or have your new physician request records through our fax at 602-391-2643. As always, in a medical emergency please contact your local emergency department.
